Grant portal proposal
One secure, phone-friendly place for EMS providers to apply, upload, sign and submit, with the records your audits will ask for.
- Prepared for
- Kerry Moody and Sara Crochet, Arkansas Ambulance Association
- Stage
- Stage 1: EMS-CONNECT grant portal
The goalEvery eligible provider applies on time, and every application is easy to find later.
We will build a grant page and application on a Matmon-based platform that links from the Grants button on your current Squarespace site. Every submission lands in one organized system with a signed, timestamped copy, so Sara can pull what a committee, a state reviewer or a federal auditor asks for in minutes instead of days.
What applicants will seeRead, apply and sign from a phone.
Many directors will open this from a phone between calls. They see the key facts first so they know if the grant fits, then move through the application one step at a time. Progress saves as they go, so they can stop and come back.

You and Sara get an email the moment an application is submitted. From there you can request a missing document, see who has not opened the announcement, resend it with a new subject line, and export a full record for the committee or an audit.
Our recommendation
Stage 1: the EMS-CONNECT grant portal.
This is everything needed to open applications for EMS-CONNECT: the program page, the online application with electronic signature, the system behind it, and the launch announcement to your providers. Nothing else is required to get started.
What the $5,000 includes
EMS-CONNECT program page
A clear page for the grant, laid out like the Rapides Foundation example you liked: overview, who can apply, key dates, what to have ready before starting, guidelines to download and common questions. Room to add your grant consultant's slide deck and the application webinar when it is recorded.
Online application
Your current fillable PDF rebuilt as a step-by-step online form that works on phones, tablets and computers. Applicants can save and come back, upload supporting documents, and still download a paper version if they prefer.
Electronic signature with a timestamp
Applicants sign online. Each submission produces a signed PDF with a signing certificate showing the date and time, signer, email and IP address. A copy goes to the applicant and one stays with the Association.
Submissions into one system
Every application flows into Portal as a provider record, tagged by program and county, with its documents attached. You and Sara are notified on every submission, and you can request more documents from the same record.
Provider list and launch announcement
We load your Department of Health list of licensed EMS providers and send one launch announcement with a link to the portal. You can see who opened and clicked, and resend to anyone who did not open it with a different subject line.
Reporting and access
Status reports and exports for committee scoring, state reporting and federal audits. You decide who on your team, committees or board can log in and what each person can see, so provider data stays private.
How it works with what you already have
Your website
Your Grants button links to the portal. Nothing else on the site has to change for Stage 1.
Updates
For the first month after launch, changes are included: a new notice for applicants, a revised date, a clearer question. Just tell us what you need.
TechLogic
We will coordinate with TechLogic so Portal fills the gaps around your new subscriptions and AI tools instead of repeating them. It works with Canva.
New features beyond the scope above, and new grant programs after Stage 1, are quoted separately before any work begins.
Why the portal matters
A large award, a short window and a lot of records to keep.
The grant moves fast once vendors are chosen, and the reporting continues long after the money is spent. The portal is built for both.
EMS-CONNECT award to the Association through Arkansas's Rural Health Transformation Program (Arkansas Democrat-Gazette).
Arkansas counties fall under the 25,000 population line for first-year eligibility (Arkansas Democrat-Gazette).
Year 1 program dollars must be spent by September 30, 2027 (Arkansas Legislature briefing).
Data reporting from participating EMS agencies, with a third-party evaluator reviewing the program (Arkansas Democrat-Gazette).
Timeline
A preview for your grant committee first, and live when you are ready to open applications.
-
This week: sign and start
Sign online and send the $2,500 deposit. Send us the draft application, any guidelines and the grant consultant's slide deck.
-
After signing: committee preview
A password-protected preview of the program page and application on your own ClientShare link, ready to show at your grant committee meeting.
-
After committee approval: finish and test
We put in the approved application, eligibility and dates, connect submissions to Portal, load the provider list and test signing from phones and computers.
-
When vendors are set: go live
The Grants button points to the portal and the launch announcement goes out. The second $2,500 and the $150 monthly Portal begin at launch.
-
First month after launch: updates included
We make the changes you need while applications come in, and set up the reports your committee and auditors ask for.
Approval
One signature, and we start this week.
Signing approves Stage 1, the $5,000 grant portal build and the $150 monthly Portal described in this proposal. The signing page you use is the same kind your applicants will use, so you will see the timestamped certificate for yourself.
